How do I know if I need to submit a Proof of Claim Form or Reinstatement Form?

  • If you are an existing customer seeking a refund, you do not have to submit the claim form to the Receiver to obtain that refund.  Your refund will be computed by the Receiver and sent to you. 
  • If you are a former customer seeking a refund, you must complete the claim form and submit it to the Receiver by July 15, 2021. Please note that if you remained under contract with Terminix in 2019 and cancelled your contract in 2020, your refund for 2019 will be computed by the Receiver and sent to you automatically. You may be eligible for additional relief, however, relating to your experience in 2020 and to obtain that additional relief you must complete the claim form and submit it to the Receiver by July 15, 2021 .
  • If you are an existing or former customer seeking a property damage claim, you must complete the claim form and submit it to the Receiver, but only after you have first tried to resolve your dispute with Terminix. 
  • If you are an existing or former customer and you have a dispute of any kind with Terminix, you may submit that dispute to the Receiver by completing a claim form by July 15, 2021, but only after you have first tried to resolve your dispute with Terminix. 
  • To obtain this relief, you must submit either
    1) a Reinstatement Form by July 15, 2021.
    or
    2) a Proof of Claim Form by July 15, 2021.
